Electronic copies of all Practice Sample materials are required to be directly uploaded according to instructions. No other submission formats will be accepted.

ABPA uses secure electronic systems to collect and store application materials and takes reasonable measures to protect applicant information.

However, applicants are responsible for removing all protected health information and personally identifying information from any clinical reports, case materials, or work samples submitted to ABPA. Submission of materials containing identifiable patient information is prohibited.
